Episode 4

Peyton is an apparently healthy toddler who suddenly falls ill and is rushed by helicopter to the Hopkins pediatric intensive care unit. After examining the child, doctors present his distraught parents with two bleak options: let him deteriorate and provide palliative care or attempt a risky heart transplant if one becomes available.

By his own admission, vascular surgeon Tom Reifsnyder is not cut in the Hopkins mold. Irreverent, chatty, and candid about his marital travails, Reifsnyder's surgeries tend to be comic relief sessions during which he interrogates his residents on everything from their romantic lives to the kind of cars they drive.

Ann Czarnik returns in this episode to escape Baltimore and the gruelling routine of the Hopkins ER, embarking on a roadtrip that will lead her home to the hill country of West Virginia.
